Does anyone know how to read my script? - Buy Cheap Glasses Online, Prescription Glasses Frames Expert

I just have my eyes checked and I got my script today. I wanted to order online eyeglasses but I stopped on Rx fill in form. I don't know how to enter my script. What I have are the following:

DV:
OD -350 -25 x 102
OS -325 -50 x 100
NV +1.25
Please let me know how to enter my script.

On the online Rx form enter your script this way:

EYE SPHERE CYLINDER AXIS ADD
OD -3.50 -0.25 102 +1.25
OS -3.25 -0.50 100 +1.25

Then enter your PD or pupil distance.
