please help
The prescription from my eye doctor show two other things
OD - lens type DVO and Dis/Near 64/61 and the 64 is circled where do I put these things or are they required

DV or Distance vision, it has OD and OS numbers on Sphere. Cylinder and Axis if you have astigmatism
NV is near vision determines reading power, it is always a +. Also for Both eyes OD and OS. PD Distance is the higher number while PD near is the lower or smaller number.
