OD - lens type DVO and Dis/Near 64/61 and the 64 is circled where do I put these things or are they required |

DV or Distance vision, it has OD and OS numbers on Sphere. Cylinder and Axis if you have astigmatism
NV is near vision determines reading power, it is always a +. Also for Both eyes OD and OS. PD Distance is the higher number while PD near is the lower or smaller number. |
