Here I am again, I got an email from you all asking me to verify my
prescription. I tried to send a copy of the prescription, but I am
having trouble sending email. Here is the prescription as I read it off the
form:
OD right sphere: +2.00 cylinder: -0.50 axis: 105
OS left sphere: +2.75 cylinder: -0.25 axis: 090
near PD right: 31.5 Left: 32.5
I am sorry about the confusion. Hope its okay to send it here...

If you are having trouble sending an email you can also send it through fax,
their fax number is 831-597-5018.
